Introduction
Quality of Life is about your position in life, within your own community and in terms of your personal beliefs, dreams and values. The quality of your life is the result of the interaction between you, your goals and your expectations and the experiences you have when you interact every day with the people, places, things, and services that you encounter.
This questionnaire is called the Quality-of-Life Impact Assessment Tool - the QIAT. It is designed to allow a person who is receiving a service which is intended to enhance their quality of life to give feedback on the extent to which the service has succeeded in this.
The figure below provides you with an overview of the areas of quality of life that the QIAT asks you to consider, which includes:
- Individual Empowerment (Personal Development and Self-Determination).
- Social Participation & Active Inclusion (Interpersonal Relationships, Rights & Citizenship, Employability and Community Participation).
- Wellbeing (Emotional, Physical and Material).
As you work through the QIAT, you will be asked to think about whether the service, which you have been receiving, has made any difference to you in each of the areas listed in the figure.
The QIAT presents you with a short description of each area so that you are clear about what it covers and asks you whether you agree or disagree with a set of statements about the impact of the service you received.
