Introduction

Quality of Life is about a person’s position in life, within their own community and in terms of their personal beliefs, dreams and values. The quality of a person’s life is the result of the experiences they have when they interact on a daily basis with the people, places, things, and services that they encounter and how these meet their goals and expectations.

This questionnaire is called the Quality-of-Life Impact Assessment Tool - the QIAT. It is designed to allow a person who is receiving a service, or someone acting on their behalf, to give feedback on the extent to which a service they are receiving has succeeded in enhancing their quality of life.

The figure below provides you with an overview of the areas of quality of life that the QIAT asks you to consider, which includes:

  • Individual Empowerment (Personal Development and Self-Determination).
  • Social Participation & Active Inclusion (Interpersonal Relationships, Rights & Citizenship, Employability and Community Participation).
  • Wellbeing (Emotional, Physical and Material).

As you work through the QIAT, you will be asked to think about whether the service in which the person on whose behalf you are acting has made any difference to them in each of the areas listed in the figure.

The QIAT presents you with a short description of each area so that you are clear about what it covers and asks you whether you agree or disagree with a set of statements about the impact of the service the person has received.
